A consent letter is a formal request made by a parent or legal guardian which is attached to the back of their child’s medical records so there is no confusion about whether they have their permission for routine physician office visits, hospitalizations, or surgical procedures. In order to write a consent letter you’ll need to contact the school nurse for their requirements and fill out school-specific forms in which you specify your intention to voluntarily waive your minor child’s statutory rights that would otherwise apply when administering his/her care.
Consent letter example:
Dear School,
We are requesting a letter from you in regards to our emergency contact. We, [insert names here], have been receiving threatening letters from [insert perpetrator’s name]. We would like to be permitted to choose a different emergency contact for this offender. If it is not possible that we may replace the current emergency contact for this offender in the school system, then we would appreciate an update on why not. Thank you!
